We start by understanding Who is involved, What happened, When it started, Where support is needed, and Why it escalated. Then we turn that clarity into a shared work plan—specific steps, timelines, and responsibilities—so progress is measurable and support stays coordinated.

Identify the veteran, family members, providers, and supports involved—plus the best way to reach everyone.

Clarify the immediate crisis and the underlying barriers (housing, income, health, legal, benefits, transportation).

Map the timeline: when the issue began, key turning points, and what needs to happen first to reduce risk.
Where & Why
Pinpoint where help is needed most and why the situation escalated—so the plan addresses root causes, not just symptoms.

Early on, we check in often. As things stabilize, follow-ups taper to monthly, quarterly, and annual touchpoints—helping veterans stay housed, stay connected, and catch issues before they become crises again.
